You can buy professionally made "sounds", which are specially made stainless steel bars in varying diameters, designed to enlarge the urethral opening.
There are some guys that have very narrow openings, and using "sounds" is one way of enlarging without cutting. I don't think "sounding" makes the hole permanently larger, just stretches it.
